2010-2011 Admissions Checklist for Prospective Parents

 

 

Visit to School

 
  Parent visit:  includes an individual meeting and tour with the Director of Admissions

 

Application Materials

 
  Application: signed and dated, due January 15, 2011

 
  Non-refundable application fee of $150.00

 

 
  Testing: A full psychoeducational evaluation which includes a WISC- IV or a WPPSI-III (must be within the past 2 years)

 

 
  Required Forms

Transcript Release sign and submit to your child’s current school
Common Referral Form submit to your child’s current school
Math Evaluation submit to your child’s current math teacher after November 1
Tutorial Referral Form submit form to the tutor or speech pathologist working with your child (if applicable)

 
  Additional Testing: Speech and Language, Occupational Therapy (if applicable) 

Shadow Visit

 
  We will arrange a Shadow Visit for your child following submission of an application.  This is an opportunity for the child to spend a full day experiencing Odyssey with a current student acting as host.

 

Admission Decisions will be mailed on Friday, March 1, 2011
